The layout for the printer and PDF Maker are the same. The problem is that WORD will reflow the document to best fit the current printer, but not necessarily reset all of the bookmarks accordingly. If you change the printer to the Adobe PDF before you create the PDF with PDF Maker (create PDF or Save As PDF), then you should be able to check the DOC file before the PDF creation to be sure it will save things correctly. The reflow will change the length of lines, paragraphs, and paging through an entire document. The number of pages can even change. This reflow feature is a characteristic of most word processors, one of the differences between a word processor and a publishing program like InDesign or Publisher.
